Patient Referral

Patients can be referred for physical therapy evaluation and treatment by physicians for any variety of orthopedic or neuromuscular conditions.  Dentists can make referrals for temporomandibular joint disorders, and podiatrists for foot and ankle problems.

Without a referral, patients can be seen by the Physical Therapist for evaluation, consultation, and education.  If ongoing physical therapy is indicated, treatment will be continued in conjuction with a physician, dentist or podiatrist.

Getting Started          

STEP 1:   Call our office to schedule an appointment. (248) 338-7600

STEP 2:   Have your insureance information ready when you call:
                        a.  Name of your provider
                        b.  Provider phone number
                        c.  Group or individual ID number

STEP 3:   Have your doctor's prescription available.  This will include the name of your referring physician and their phone number.  If you have not seen a doctor yet, feel free to contact us and we will assist you in this process.

Please contact our office in person or by phone to arrange for you first appointment.  All new appointments are scheduled within 24 to 48 hours. Ongoing appointments are scheduled at the patient's availability with early morning, evening, and Saturday appointments available to accommodate all schedules.

Your First Appointment

During your first appointment you will be evalutated by a physical therapist.  The therapist will ask you questions regarding your symptoms and history.  Please let the therapist know if there are any questions that you do not understand or if you have any special concerns regarding physical therapy.

The therapist will take objective measurements of movement, strength, reflexes, gait, and balance.  In order to visualize the areas needing threatment, please bring or wear loose fitting clothes such as shorts, tank tops or T-shirts.  A gown can be provided if necessary.  The first visit will likely take about 1 to 1 1/2 hours.

As determined by the evaluation, a physical therapy program will then be designed based on the individual needs of the patient.  Any and all subsequent visits will be with the same evaluating physical therapist, and all visits will take place in a private room.
